This registration form allows you to process multiple registrations on one order IF you will pay for all of them with the same payment type, e.g., credit card. Once each registration is processed you will have the option of adding registrations to your order. Then when you check out, all registrations will be tallied for a single payment for all the registrations submitted during this ordering session.
If you want to register more than one person and use a different payment type for each of them, please complete each registration, pay, and then quit your browser and start over. Closing the browser window will not have the desired effect. Sorry for the inconvenience. We are working on a solution that allows you to register more than one person and then use a difference credit card for each of them without having to quit your browser to do so.

| Our organization is a CUG member and I wish to pay our CUG Membership fee for FY 2010 as a part of my CUG 2010 registration fee. I am or have been authorized by our CUG Installation Delegate to make this payment for our CUG site.

The total due for your registration will be calculated using the data above. You will receive a registration confirmation via e-mail which will contain both your registration and payment information.
All registration fees are due on or before the start of the conference. If this is not possible, please contact the CUG Office to arrange a mutually acceptable payment date. |

Cancellation/refund policy: Conference registration cancellations must be received by the CUG Office at the address above before
April 22, 2010. All registration fees will be refunded (less a $25 handling fee) if the cancellation is received by this date.
